Leonardo Piatti, 39, died after leaping from Monte Brento in the Trentino area, Rai News reported.
He took off from the mountain edge at around 1,200m and failed to open his parachute in time to avoid slamming into rocks. Onlookers phoned rescuers who arrived by helicopter, but were unable to save Piatti.
He was visiting the area from his home in Trebbioantico di Pesaro, central Italy, with his wife and friends when the accident happened.
The Monte Brento area is a popular spot for extreme sports, but three people have died this year alone, Rai News said.
Base jumping has been hit by a series of tragedies this year, with deaths in France, Spain, Switzerland and Spain.
